Job Summary
As a Social Worker with Northfield Hospital Clinics, you will provide services to address the needs of patients and family members in the Northfield Hospital through psychosocial assessment, discharge planning, emotional support, and providing resources.
Key Accountabilities:
- Provides psychosocial assessment for patients and their families to determine needs and appropriate services for all units of the facility including, medical /surgical, emergency room, birth center, and clinic. This position is located in Northfield and does not include travel to other clinic locations.
- Provides social work services including supportive counseling, discharge planning, and community resource planning. Facilitates referrals for follow-up care.
- Demonstrates the ability to work effectively with clients of all ages and with diverse cultural needs.
- Develops and maintains a thorough knowledge of referral resources throughout the service area.
- Works closely with members of the interdisciplinary health care team in development of plan of care.
- Performs other related duties as assigned. Work is fast-paced and busy.
Qualifications:
- Licensed by the Minnesota State Board of Social Work, Master's level preferred.
- 2 years of social work experience in a health care environment preferred, including hospital or nursing home.
- Required license supervision will be provided free on-site for recent BSW or MSW graduates.
Schedule:
- 48hrs/2wks
- Shift: 8:00am-4:30pm
- Three days per week; Wednesday, Thursday, Friday preferred, with an opportunity to discuss another schedule if needed. Once set, the schedule will remain the same every week.
