We are currently searching for a MDS Coordinator to join our team. As an MDS Coordinator, you will be instrumental in giving your team the knowledge they need to care for each resident's unique needs.
The role involves developing, monitoring, auditing, and modifying each resident's care plan for their individual needs and goals, performing resident assessments, and assisting in the discharge process.
Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Responsibilities:
- Develop, monitor, audit, and modify each resident's care plan for their individual needs and goals.
- Perform resident assessments and assist in the discharge process.
- Ensure residents receive high-quality care by providing knowledge, skills, and attention to detail.
Requirements:
- An unencumbered State of Iowa R.N. License or LPN certificate.
- Graduate of an accredited nursing program and C.P.R. Certification.
- Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting, and (1) year of experience as a C.N.A. is preferable.
