Job Description
The MDS Coordinator plays a critical role in ensuring our residents receive the best possible care. This includes developing and implementing individualized care plans, performing resident assessments, and collaborating with healthcare professionals to ensure effective care delivery.
Key Responsibilities
- Develop and maintain accurate and up-to-date resident care plans.
- Conduct resident assessments and provide recommendations for care planning.
- Collaborate with healthcare professionals to ensure effective care delivery.
- Participate in quality improvement initiatives to enhance resident care.
Required Skills and Qualifications
- R.N. License or LPN certificate from the State of Iowa.
- C.P.R. Certification.
- Prior experience as an R.N. in a Skilled Nursing Facility setting (preferred).
- Prior experience as a C.N.A. (preferred).
