NorthKey Community Care is seeking a full-time Case Manager for its Intellectual and Developmental Disabilities (IDD) Crisis Program to monitor crisis support services and facilitate stabilization of crisis situations for clients with intellectual and developmental disabilities (IDD). A Bachelor’s Degree in a human services field is required.

Case Manager for Intellectual & Developmental Disabilities Crisis Services - Qualifications
- Bachelor’s degree is required, preferably in a human services or behavioral health field of study.
- Experience working with individuals with intellectual and/or developmental disabilities is strongly preferred, but not required.
- Excellent written and verbal communication skills.
- A valid driver's license and the ability to operate a motor vehicle.
Case Manager for Intellectual & Developmental Disabilities Crisis Services - Responsibilities
- Collaborate with IDD Crisis Services Team Leader to monitor crisis support services including crisis responders.
- Utilize person-centered planning techniques to facilitate prevention and stabilization of crisis situations for clients with intellectual and developmental disabilities.
- Involve the family, guardian, and providers in planning by reviewing and coordinating the plan of care with them when appropriate and with client’s consent.
- Identify and implement appropriate long term supports and services, including residential placement for IDD individuals as needed.
- Collaborate with IDD Crisis Services Team Leader to develop a mobile crisis response system for IDD individuals.
- Assist to identify, train, and monitor responders to assure appropriate emergency response and crisis stabilization.
- Develop ongoing crisis resources within local community to meet immediate and ongoing needs of individuals.
- Complete all necessary documentation and reports.
- Collaborate with IDD Crisis Team to provide relevant staff training relating to crisis prevention and response, person-centered planning, and positive behavioral supports within inter-agency training coalition.
- Provide outreach to community partners to educate and to enhance crisis prevention and response services within region.
- Ensure availability for crisis intervention/emergency referrals, as well as scheduled on call hours.
- Maintain HIPAA compliance and appropriate client confidentiality.
