Team Leader for Intellectual & Developmental Disabilities Services - Summary

NorthKey Community Care is seeking a full-time Team Leader for its Intellectual and Developmental Disabilities (IDD) Case Management Program, specifically its Supports for Community Living (SCL) Medicaid Waiver program and its Home and Community Based (HCB) Medicaid Waiver program. The Team Leader will guide and direct a team of case managers to engage and support clients with intellectual and developmental disabilities enrolled in the SCL and HCB waiver programs. A Bachelor’s Degree in a human services field is required, and at least one year working with individuals with developmental and intellectual disabilities is required.

Team Lead for Intellectual & Developmental Disabilities Services -Qualifications

  • Bachelor’s Degree in acceptable human service field of study is required.
  • Relevant knowledge, training, and at least one year experience working with individuals with intellectual and developmental disabilities (IDD) is required.
  • Kentucky Medicaid Waiver knowledge is required.
  • Good written and verbal communication skills.
  • Supervisory experience preferred, but not required.

Team Lead for Intellectual & Developmental Disabilities Services - Responsibilities

  • Guide and direct others in the engagement of clients with intellectual and developmental disabilities (IDD) in structured, collaborative relationships, which empowers clients to gain necessary skills in a variety of areas related to recovery and self-management of their developmental and/or intellectual disability.
  • Oversee and coordinate Kentucky Medicaid Waiver services for the Support for Community Living Waiver (SCL) and Home and Community Based Waiver (HCB) programs.
  • Guide and direct others in addressing functional challenges of IDD individuals served.
  • Assist supervisees in identifying and addressing risks and current or potential gaps in care in order to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, or loss of independence in vocational, academic, or social functioning.
  • Assist in the development and guidelines for promoting environmental and psycho-social health and maximize independence to improve and maintain lifelong wellbeing and functional status.
  • Develop process and procedures related to SCL and HCB services.
  • Complete initial and/or annual and other mandatory trainings as assigned related to care coordination and case management.
  • Collaborate with other members of the care team involved in the client’s case.
  • Maintain HIPPA compliance and appropriate confidentiality.
  • Assist in the education of IDD clients and/or families and significant others on resources to assist with identified needs.
  • Adhere to organizational policies regarding documentation, training, and appropriate/timely services to clients.
  • Monitor documentation of supervisees.
  • Monitor time and attendance of supervisees.
  • Prepare reports related to the provision of services.
