Overview:
The Community Support Professional is a key role focused on assisting individuals with long-term mental illness. This position operates under the guidance of the Clinical Coordinator and Program Director, aiming to enable these individuals to live and engage within their community.
Key Responsibilities:
Treatment Plan Development and Implementation:
Collaborate with consumers to develop and implement personalized treatment plans.
Conduct thorough individual needs assessments.
Support Coordination:
Arrange and coordinate essential supports for maintaining individuals in community settings.
Provide symptom and medication monitoring.
Facilitate access to psychiatric and psychological services.
Assist in finding employment, education, residential, medical, dental, financial, family, and social/recreational opportunities.
Record Maintenance:
Keep detailed consumer-related records, including files, progress notes, and financial documents.
Skills Training:
Provide training in self-care, household management, financial management, and recreational activities as required.
Supervision and Coordination:
Supervise and coordinate activities of any supportive home staff.
Offer case management to ensure continuity and appropriateness of services.
Crisis Intervention:
Provide crisis intervention services both via telephone and in-person.
Service Coordination:
Coordinate with various agencies and act as the primary contact for families or primary caregivers.
Participate in commitment hearings and collaborate with local law enforcement and the District Attorney's office.
Advocacy:
Advocate on behalf of the consumer.
Schedule and conduct regular reviews of treatment plans.
Team Collaboration:
Work effectively as part of a team, including providing backup for absent team members.
Transport individuals to medical appointments and other community services as necessary.
Psychotherapy:
Provide individual or group supportive psychotherapy (Master’s level professionals offer psychotherapy).
Professional Development:
Participate in job-related training and staff meetings.
Engage in program planning and implementation.
Communicate training needs.
Additional Duties:
Perform miscellaneous responsibilities as assigned.

REQUIRED TRAINING AND EXPERIENCE:
Bachelor’s Degree in behavioral science with 1000 hours experience with persons with long term mental illness. Must possess a valid Wisconsin driver’s license and insured vehicle. Or, Master’s Degree in behavioral sciences, plus 3,000 hours experience with person’s with long term mental illness, or 1500 hours working in a Community Support Program.
