We are seeking a qualified and motivated Social Worker to join our team. In this role, you will support participants by addressing their psychosocial needs, coordinating care plans, and collaborating with an interdisciplinary team to promote their overall well-being.
As the social worker, you will be responsible for:
- Completing social work assessment during initial, semi-annual, annual and change in condition evaluations.
- Providing care to caseload of PACE participants in a variety of settings including the PACE day center and participant home.
- Coordinating with the interdisciplinary team to develop a comprehensive care plan centered around the participant.
- Determining participant and family needs related to social support, financial support, mental health needs, counseling and housing.
- Providing individual and group counseling to participants and their families as identified.
- Coordinating the completion of participant’s health care wishes and advance directives in cooperation with their primary care physician, the participant and their family.
- Coordinating with mental health-related providers including drug and alcohol treatment to arrange appointments and share pertinent information.
- Participating in Quality Improvement activities as requested.
- Petitioning for guardianships when deemed appropriate by the interdisciplinary team.
Understanding and following Northland PACE Program policies, procedures and protocols.

Employment Specifications:
- Education/Experience: Master’s Degree from a school of Social Work accredited by the Council on Social Work Education.
- Licensure, registry or certification required: Must possess current, valid SW licensure in the state of ND. Must maintain current licensure/certification within scope of practice.
