Summary/
Objective
The Pediatric Care Coordinator is responsible for coordinating care to achieve the best possible health outcomes for children ages newborn to 18 years. The Pediatric Care Coordinator is a core member of the Pediatric Care team and the Pathways department. The person in this role will provide one on one patients visits with families, providing education, support, referral assistance, and any other patient need. The Pediatric Care Coordinator also works closely with the Women’s Health Care Coordinator to ensure proper transitions of care.
Essential Functions
Completes patient visits with all newborns to assist families with unmet/unfulfilled needs
Assists with scheduling health & wellness services (including, but not limited to well-child visits, immunizations, annual physicals/screenings, dental examinations and cleanings, vision exams, behavioral health, etc.)
Assists with referrals to WIC, Breastfeeding support, and external specialists
Assists patients in obtaining durable medical equipment, such as helmets, braces, crutches, nebulizers, etc.
Provides age-appropriate education, including immunization, safe sleep and milestones.
Helps community members during Outreach events and services
Completes car seat safety checks, installations, and safety checks.
Complete & maintain timely & accurate documentation in EHR programs
Other duties as assigned by the Pediatric Care Coordinator Supervisor and/or Care Coordination Manager

Work is performed in an office environment. Involves frequent personal and telephone contact with patients, physicians, and other healthcare personnel. Work may be stressful at times. Interaction with others is constant and interruptive.
Travel
Travel outside of Northshore locations will be rare for this position.
Qualifications
Required Role Qualifications
Minimum required education per state of Indiana/HRSA
Ability to obtain CHW Certification within 1 year of hire
Ability to obtain Child/Passenger Safety Technician Certification within 1 year of hire
Preferred Role Qualifications
One year of experience working within a healthcare setting.
1-2 years of experience working with children and families
