Qualifications
- Current Colorado Occupational Therapist license
- Availability M-F, 8am-5pm; weekends as needed
- Preferred prior home care experience
- Minimum one year recent OT work experience
- Ability to work independently and manage time
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; EMR experience preferred
- Conduct home visits and thorough patient evaluations
- Develop individualized intervention plans
- Assess home environments for hazards
- Identify equipment needs and adaptations
- Revise care plans and collaborate with nursing
- Educate patients and families on care and safety
- Document patient care accurately using a tablet
