Title: Client Care Coordinator

The Client Care Coordinator plays a vital role in ensuring individuals seeking support for substance use disorders receive prompt, compassionate, and professional assistance. This role is the first point of contact for potential patients, current patients, referral sources, and alumni. The Client Care Coordinator is responsible for answering incoming calls, providing accurate information about services, and coordinating treatment plans while maintaining a supportive and empathetic approach.
1. Telephone and Communication Support
- Answer incoming calls with professionalism and empathy, providing accurate information about the facility’s programs and services.
- Serve as the initial point of contact for potential new patients, guiding them through the intake and admissions process.
- Respond to inquiries from referral sources, ensuring timely and clear communication about patients’ needs and progress.
- Maintain ongoing communication with current patients, offering support, reminders, and updates as needed.
- Act as a liaison for alumni, fostering engagement and support within the Northstar recovery community.
2. Patient Coordination
- Schedule initial consultations and coordinate treatment appointments for new and existing patients.
- Assist in collecting and verifying patient information, including insurance details and consent forms.
- Collaborate with the clinical and administrative teams to ensure a seamless patient experience.
3. Referral Source Engagement
- Build and maintain relationships with referral sources, including healthcare providers, social workers, and community organizations.
- Provide updates to referral sources while adhering to confidentiality and HIPAA regulations.
4. Administrative Duties
- Document all communications and interactions in the patient management system.
- Monitor and manage the facility’s general email and voicemail inboxes, ensuring timely responses.
- Generate and distribute reports related to patient inquiries, admissions, and follow-ups as requested.
5. Alumni Support
- Assist with planning and coordinating alumni events and support group activities.
- Maintain contact with alumni to provide ongoing encouragement and access to resources.
- High school diploma or equivalent; additional education in behavioral health, social work, or related fields is a plus.
- Experience in a customer service, administrative, or healthcare-related role.
- Excellent communication skills, both verbal and written, with a compassionate and nonjudgmental demeanor.
- Strong organizational skills and the ability to manage multiple tasks simultaneously.
- Proficiency with word processing and spreadsheets. Experience with patient management systems is a plus.
- Familiarity with substance use disorders, treatment options, and recovery processes preferred.
- Ability to handle sensitive and confidential information in compliance with HIPAA and other regulations.
- The position operates in an office setting with frequent phone and computer use.
- Requires flexible hours to meet patient or organizational needs.
