Job Description

The Clinical Pharmacist, Ambulatory Pharmacy Services, plays a key role in supporting the Senior Director of Pharmacy Shared Services Integration in optimizing pharmacy services delivery across ambulatory and physician practices. This position focuses on the clinical aspects of shared services integration, ensuring alignment with operational, financial, and regulatory requirements. The Clinical Pharmacist will collaborate closely with the Senior Director to bridge clinical pharmacy expertise with the needs of diverse practices, enhancing patient care quality and medication safety.

Job Responsibility

  • Develops and monitors the hospital’s policies/procedures regarding utilization of medications in collaboration with the medical staff, Pharmacy, and Therapeutics Committee; monitors medication utilization effectively and intervenes when necessary to ensure compliance.
  • Monitors pharmacists activities regarding utilization surveillance and provides instruction/education as necessary; maintains necessary and appropriate records regarding the effectiveness of these drug therapy regimens and provides information as required.
  • Ensures the safe and effective use of medication; guides staff in the provision of clinical services.
  • Provides necessary, requested information to management for consideration in the staff’s performance appraisals; collaboratively, coordinates/implements educational training and competency programs for the staff and pharmacy students.
  • Provides educational programs for health care staff regarding medication use; provides training as measured by the success of the staff to accomplish identified goals and improve patient care; incorporates professional concerns into the program.
  • Coordinates activities with the assigned student preceptor and monitors the student’s activities and evaluates performance; initiates and implements programs.
  • Coordinates and participates in the development of the hospital’s Formulary and related activities; contributes to the Formulary review process.
  • Initiates/coordinates MUEs for P&T presentation; discusses findings.
  • Performs all duties of a licensed registered Pharmacist as required by the leadership and as identified by the ability of the department to effectively provide comprehensive service; maintains compliance with established procedures, policies, and laws in performing the duties of a licensed, registered staff pharmacist.

Key Responsibilities:

  • Clinical Program Development and Implementation: Collaborates with the Senior Director to develop and implement clinical programs and initiatives that support shared services integration, including medication therapy management, disease state management, and patient education programs.
  • Clinical Expertise and Support: Provides clinical pharmacy expertise to ambulatory practices, including drug information, medication recommendations, and patient counseling. Supports the development and implementation of standardized medication protocols and guidelines.
  • P&T Committee Support: Assists the Senior Director in preparing and presenting initiatives to the ambulatory Pharmacy & Therapeutics (P&T) committee. Participates in P&T meetings and provides clinical input on medication formulary decisions. Supports communication and collaboration with system-level cancer care, pediatrics, and local P&T committees.
  • Quality Improvement and Medication Safety: Contributes to quality improvement initiatives related to medication use and safety in the ambulatory setting. Participates in medication safety event reviews and implements strategies to prevent future events.
  • Collaboration and Communication: Works closely with physicians, nurses, and other healthcare professionals in ambulatory practices to optimize medication therapy and patient outcomes. Collaborates with procurement on medication standardization initiatives and formulary management.
  • Regulatory Compliance: Ensures that all pharmacy services are delivered in compliance with relevant federal and state regulations. Stays up-to-date on changes in regulations and implements necessary updates to policies and procedures.
  • Data Analysis and Reporting: Collects and analyzes data related to medication use and patient outcomes to identify areas for improvement and measure the impact of clinical programs. Prepares reports and presentations to communicate findings to stakeholders.
  • Training and Education: Provides training and education to staff and other healthcare professionals on new medications, clinical guidelines, and best practices in medication therapy management.

Job Qualification

  • Bachelor of Pharmacy with 1 year internship required, or equivalent combination of education and related experience.
  • Current License to practice as a Pharmacist in New York State required, plus specialized certifications as needed.
  • 1-3 years of technical experience, required.
