Job Description
Provides individual, and/or group work services to patients and/or their families in various health care settings. Utilizes psychosocial assessment skills to deliver social work services.
School-Based Health Center
RESPONSIBILITIES:
- Works as member of an interdisciplinary team
- Performs related duties, as required.
- Conducts mental health assessments
- Creates treatment plans
- Provides individual therapy sessions both short and longer term
- Implements group work
- Handles and/or acts as a resource for difficult or complex issues related to social work/ mental health care
- Manages crises
- Provides collateral family sessions
- Performs psychosocial assessment of patients and families.
- Formulates and implements psychosocial treatment plans.
- Identifies unmet psychosocial needs and develops treatment to address them.
- Communicates ongoingly with school personnel and acts as mental health resource
- Collaborates with SBHC medical team for care coordination
REQUIRED EXPERIENCE AND QUALIFICATIONS
- Master of Social Work required.
- Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
- Minimum of one (1) year of clinical work post MSW graduation required.
- Prior experience with adolescents is preferred.
- Fluent in Spanish required and able to conduct therapy sessions in Spanish
- Ability to communicate effectively with patients and families, while being sensitive to cultural diversity.
