Role Description
Provides full-service case management for participants in the shelter program. Demonstrates compassion and customer service while leading the navigation team for shelter participants. Responsible for assessing, guiding, and assisting shelter participants from entry to graduation from the program, including initial intake, identifying goals, developing a case plan, resolving issues, and providing resources. Maintains data collection, reporting and record keeping in compliance with local, state and federal regulations. Ensures privacy and confidentiality of information, protects the assets of the organization and acts with ethics and integrity. Aligns with and models the policies and values of Northwest Coastal Housing, CDC.
This is a full-time on-site role as a Navigation Manager located in Lincoln City, OR. The Navigation Manager will be responsible for overseeing navigation services, coordinating resources, and providing assistance to individuals and families in accessing various programs and services.
Qualifications
1. Services @ CPR
a. Grant writing participation, following up reports & tracking to close out the grant.
b. Policy & Procedures, assist writing new process and procedures policies, ensure leadership review and implementation.
c. Shadowing, mentor new navigation team members
d. Convening-Leadership huddles, oversight of agency huddles
2. Data
a. Oversight of data tracking systems, (HIWAY, Measure 110, Fuse, Samaritan, etc.)
b. Training all new staff members
3. Training
a. Housing- Shelter, Landlord & Grant laws
b. Leadership training, ongoing training for leadership staff. Actively participate in community leadership meetings and translate the information to the navigation team.
c. Trauma Informed- workplace. Assist HR with trauma informed workplace training and ensure it is modeled, and train to comply with trauma informed care education.
Job Description
Performs assessment and intake of all individuals and families applying for shelter.
Meets with program participants Monthly to help them set goals and develop a case plan then follows-up and monitors their progress.
Oversees Navigation team assists participants to meet their goals by helping them to resolve problems and remove barriers.
Uses knowledge of community agencies and services to connect participants with necessary resources.
Ensures participants understand and comply with program rules and expectations.
Works collaboratively with other team members to avoid issues and promote harmony with residents.
Maintains mandated data collection and reporting as well as records of services provided. Issues exit notices and approves extensions consistent with policies.
Provides de-escalation and crisis intervention training services as needed.
Oversees and delegates to volunteers or employees who assist the Navigation Manager

Salary : $60,000 - $68,000
