Community Health Worker Navigator
This position will focus on providing healthcare education and outreach services to farm workers and their families. Connect farm workers and their dependents to wraparound services such as food, housing, transportation, childcare, etc.
What You’ll be Doing
· Provide healthcare education and outreach services to farm workers.
· Provide participants in Clackamas, Multnomah, and Washington counties with
enrollment, redetermination, and navigation services.
· Follow up with participants as needed.
· Maintain participant documentation and enter project data.
· Regular attendance is required.
· Other duties as required.
· Follow the protocol, policies, and procedures of the department and agency.
· Maintain accurate documentation on a consistent basis.
· Complete and submit reports as needed.
· Maintain confidentiality of client information and comply with HIPAA guidelines.
· Provide culturally responsive services that are equitable, respectful, and relevant to people from a wide variety of cultures, backgrounds and orientations.
· Attend meetings and trainings as required.
· Regularly participate in professional development.
What You’ll Need
· Bilingual and bi-cultural English and Spanish is required.
· A high school diploma or equivalent is required.
· Must be a Certified OHA Assister or become certified within 6 months of taking the position.
· Have a Community Health Worker certification or become certified within 6 months of taking the position.
