Job Overview
The Community Health Nurse plays a vital role in the mobile health screening program, providing direct health screenings and education to underserved communities across Escambia, Santa Rosa, Okaloosa, and Walton counties. This field-based position requires daily travel to community sites such as health fairs, homeless outreach centers, and rural areas. The nurse will be responsible for conducting screenings, interpreting results, and connecting individuals with necessary follow-up care.
The ideal candidate is an independent, mission-driven professional with experience in community health, preventive care, and patient education.
Key Responsibilities
- Conduct on-site screenings for diabetes, hypertension, heart disease, and vision issues.
- Perform basic health assessments and educate individuals about their results.
- Identify individuals at risk and provide referrals to case navigators for follow-up care.
- Mobile Health Operations:
- Travel daily to designated screening locations using a personal vehicle (mileage reimbursed).
- Assist with setup and breakdown of mobile health screening sites.
- Ensure proper handling and maintenance of screening equipment.
- Provide one-on-one and group health education on chronic disease prevention and management.
- Offer guidance on lifestyle changes and self-management strategies.
- Collaborate with community organizations to promote health services.
- Maintain accurate electronic health records in compliance with HIPAA regulations.
- Document screening results, referrals, and follow-ups for program evaluation.
- Ensure adherence to all clinical protocols and best practices.
Qualifications & Skills
Required:
- Active Florida RN or LPN license in good standing.
- Valid driver’s license and access to a reliable personal vehicle (mileage reimbursement provided).
- Strong patient education and communication skills.
- Ability to work in a mobile setting and adapt to different environments.
Preferred but Not Required:
- Proficiency in electronic health records (EHR) and data collection.
- Experience in public health screenings, case management, or health education.
- Knowledge of community health resources and referral networks.
- Bilingual (Spanish/English) is a ptus.
