5 Star Northwest Medical Homes previously know as Springfield Family Physicians, is looking for an energetic and hardworking Primary Care Behavioral Health Consultant.

Duties Include:

The Integrated Behavioral Health Consultant (IBHC) is responsible for the diagnosis, treatment, and maintenance of a diverse behavioral health and medical population, utilizing the primary care behavioral health model of care through close collaboration with primary care providers and other team members.

The IBHC may perform psychosocial assessments, develop and implement plans of care, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and provide treatment as indicated. This position works collaboratively with patients, patient's support people, healthcare providers, insurers, community resources, and all other involved parties. A typical clinic day for the IBHC at Springfield Family Physicians is an alternating schedule of 30 minutes with follow up appointments alternating with 30 minutes of warm hand off/same day visits to optimize same day access. In our larger clinic there are two IBHCs balancing this schedule allowing for as much same day access as possible.

Primary Functions:

1. Provide consultation and assistance to medical providers on the management of behavioral health concerns in primary care.

2. Conduct brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing medical, behavioral, mental health or addiction treatment. Provide referrals for more intensive services where appropriate.

3. Provide short-term behavioral interventions to engage and motivate patients to actively manage their medical and behavioral health care. Conduct follow-up as appropriate based on assessment and treatment. Conduct group visits (dual visits with medical providers, medical group visits, and/or self-management groups) which are medically, behaviorally, or skill-based.

4. Participate in ongoing training in medically integrated behavioral health issues and provide consultation and training to primary care clinic staff in mental health and behavioral medicine topics including, but not limited to communication skills, brief interventions, counseling skills, medication use and misuse, substance abuse, health promotion, rehabilitation, and evidence based approaches to self-management support, such as patient coaching and motivational interviewing.

5. With the help of the Primary Care Behavioral Health Care Manager, establish and maintain a current listing of community resources to meet the needs of the target population. Develop and maintain collaborative working relationships with community providers. Initiate collaborative contacts, disseminate information and make referrals to the appropriate services as needed.

6. Complete accurate and timely documentation in patient's medical record.

7. Follow procedures for patient, provider, and relevant metric goals per the primary care office expectations.

8. Participate in regular multidisciplinary case conferences with medical providers. Provides case-by- case assistance and consultation on behavioral health issues affecting patients. Assists medical staff by indicating the degree and severity of psychosocial and emotional factors adversely affecting patients.

9. Complete population health projects as assigned and panel-manage patients in collaboration with other medical, behavioral health staff.

10. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

11. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all individuals with an excellent service experience by consistently demonstrating core mission and leader behaviors daily. Analyzes and recommends any needed areas of improvement/development.

12. Complies with Springfield Family Physicians Standards of Conduct.

13. Other duties as assigned.

Minimum Qualifications:

1. Licensed Psychologist or Licensed Clinical Social Worker in the State of Oregon (or eligible)

2. Ability to be credentialed with all Springfield Family Physicians contracted insurance

3. Experience working with EMR/EHR systems

4. Experience working with a diverse patient panel

5. Experience working with patients with high levels of trauma

6. Commitment to improving health status of patients

7. Good interpersonal and human relations skills are required. Must possess skills to empathize and care for a variety of patients, including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity

8. Ability to maintain confidentiality at all times in all situations and follow the laws and intent of the most current HIPAA laws and regulations and Springfield Family Physicians security and privacy policies.

Preferred Qualifications:

Previous experience using primary care behavioral health models

Previous experience working with diverse populations

Salary : $80,000 - $110,000
