What are the responsibilities and job description for the Case Manager position at Northwest Michigan Supportive Housing?

Our housing-based case managers provide in-home support and may include providing rides to appointments, development of a crisis plan, a guest policy, budgeting, and a referral to other community resources and supports, as well as assisting in housing-related goal development.
