Guided by the PassageWay Elements, we strive to create a positive environment for growth and wellness for both our clients and staff.Position Description : Northwest Transitions is part of Northwest Passage Ltd., and is licensed by the State of Wisconsin as a four bed Adult Family Home. We are contracted with St. Croix County Mental Health and provide services for adults of any age who are classified by the State of Wisconsin as having emotional disturbance / mental illness.Northwest Transitions is currently seeking to hire a part-time Activity Specialist / Mental Health Technician position to develop unique programming and activities to St. Croix County's Behavioral Health Clients. These activities will work towards outcomes stated on their Recovery Plan to help residents stabilize and integrate back into their community.Responsibilities :
- Develop creative programming which works towards psychosocial outcomes as stated on each client's Recovery Plan.
- Work as a Mental Health Technician as defined by state statutes as part of St. Croix Counties Community Support (CSP), Comprehensive Community Services (CCS), and Emergency Services (ES) programs.
- Assist NWT residents in all aspects of daily living, using these as opportunities to help residents learn and work towards independency in their communities. Examples include and are not limited to : Helping residents work through periods of emotional distress, Building meaningful professional relationships, and managing all aspects of milieu within the home.
- Communicate effectively with all program staff to assure all aspects of program are implemented.
- Monitoring of medications, making sure appropriate documentation is completed.Qualifications :
- High School Diploma or equivalent
