FULL TIME MENTAL HEALTH WORKER (WEEKENDS)
Northwest Transitions, a licensed Adult Family Home operated by Northwest Passage Ltd., is currently seeking to fill a full time position at its New Richmond location. Our goal is to help adults manage their mental health so they can transition back into their communities. Qualified applicants will have a bachelor’s degree (preferred), 1,000-2,000 hours of supervised post-degree clinical experience working with adults with chronic mental illness (preferred), ability to pass a caregiver background check and pre-employment physical, and have a valid driver’s license.
Primary responsibilities include but are not limited to:
- Oversee and exercise leadership in the development of programming that meets resident’s needs as stated on their individual service plans or recovery plans. This includes assisting other staff in developing appropriate programs and activities to meet resident’s needs.
- Act as shift leader for all part time staff working during your designated shift under leadership from the Program Coordinator. Responsibilities include, serving as a resource for part time staff, role modeling positive working relationships, monitoring documentation for the shift and make sure that it is complete and adequate, communicating with Program Coordinator facility needs, Communicating scheduling and staffing needs for planned activities, and reflecting agency values when working with the county.
- Assist other NWT staff and residents in all aspects of daily living, using these as opportunities to help residents learn and work towards independency in their communities. Examples include and are not limited to: Striving to build meaningful relationships with residents, using company vehicle to transport to community activities and appointments, preparing meals, planning and leading groups targeted toward increasing skills around daily living and self-care, identifying and leading recreational activities that cater towards individual interest and abilities, helping residents work through periods of emotional distress, and managing all aspects of milieu within the home.
- Develop, plan, and deliver services that work toward the resident’s individual outcomes as stated in their Recovery Plan
- Complete appropriate documentation about the services provided in a complete and timely manner.
- Communicate effectively with all program staff to assure all aspects of program are implemented.
- Monitor medications, making sure appropriate documentation is completed.
