What are the responsibilities and job description for the CSAC Case Manager (HEART) position at Northwestern Community Services Board?
This position is responsible for managing an assigned caseload of incarcerated adults receiving Medication for Opioid Use Disorder (MOUD). The incumbent is responsible for assessing client needs, developing, implementing, and reviewing service plans. This role is part of a collaborative initiative between the Northwestern Regional Adult Detention Center (NRADC) and Northwestern Community Services Board (NWCSB), aimed at supporting recovery, re-entry into community, and reducing recidivism.

QUALIFICATIONS:
- A combination of SUD experience or relevant education, and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties required.
- Bachelor’s degree earned from an accredited college or university in a relevant human services field required.
- At least one year of experience working with clients with substance use disorders to meet Qualified Mental Health Professional (QMHP) according to Department of Medical Assistance (DMAS) requirements.
- Certified Substance Abuse Counselor (CSAC) eligibility required.
- Must possess a valid driver’s license, a good driving record, and the ability to provide your own transportation.
ADDITIONAL REQUIREMENTS:
- May be exposed to allergenic environments from client’s homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
- May be exposed to clients with infectious diseases
- Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed
- Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles
- Prolonged periods sitting at a desk and working on a computer
- Must be able to lift/push/pull up to 25 pounds at times
SALARY: Starting at $52,788.00
