Join the NCSS Adult Outreach Team and make a difference! We are seeking a strong team player to provide case management services to adults with mental health support needs. The goal of this program is to reach out to individuals who may be reluctant to participate in care but are are at risk of unnecessary inpatient hospital admissions. Essential qualities include the ability to easily connect with individuals, navigate systems, and collaborate effectively with others. Primary duties include providing outreach case management in a variety of community-based settings. This position works closely with other teams in our Behavioral Health Division.
This position requires:
- Positive attitude, team-oriented and organizational skills.
- Reliable transportation with the ability to transport clients.
- Case management experience is a plus.
Bachelor’s degree preferred. Willing to consider Associate’s Degree or equivalent combination of related education, training and experience.
