Join our committed healthcare team as a Medication-Assisted Treatment (MAT) Wellness Counselor, where you will play a crucial role in providing comprehensive care management to patients within our private family medicine practice. The practice has a team of Nurse Practitioners who are passionate about supporting the patient and their families while treating the whole person. They are looking forward to having someone join their team to assist in navigating the complex medical and social needs for their patients. This position involves collaborating with an interdisciplinary team to enhance patient outcomes through comprehensive care coordination, health promotion, transitional care, and individual and family support services.
Responsibilities:
- Comprehensive Care Management: Identify and assess potential MAT patients, develop treatment plans and provide counseling, all while monitoring patient health status and treatment progress and designing systematic approaches for Social Determinants of Health.
- Care Coordination: Communicate with providers and facilitate referrals and coordinate with other providers for seamless care transitions.
- Health Promotion: Provide nontraditional therapy in the PCP office which includes assisting in goal setting and self-management plans.
- Comprehensive Transitional Care: Develop collaborative relationships with providers by implementing referral protocols for care transitions all with the goal of reducing hospital readmissions and facilitating community placements.
- Referral to Community & Social Support Services: Assist patients in accessing community resources.
Qualifications:
- Master's Degree (MSW, CMHC) with licensure or licensure eligibility - LICSW or LADC.
- Experience providing individual therapy - knowledge of substance use/misuse.
- Strong interpersonal and communication skills.
- Knowledge of Social Determinants of Health.
