Description

Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or Physician Assistant to work in our outpatient NMH Transitional Care Clinic.

The NMH Transitional Care Clinic’s mission is to increase access and improve health outcomes for at-risk patients with complex medical and psychosocial needs through an integrated and interdisciplinary team-based approach by providing high quality, empowering, and compassionate transitional care to meet the needs of our patients, fellow providers and community partners. Specifically, Transitional Care Clinic follow-up is for patients after an acute care crisis to help improve their health, understand how to navigate the medical system, and transition more easily to long-term primary care physicians in their communities (patients typically are followed in TC for approximately 6 months).

On call required, about 9 weeks/year. MDs and APPs rotate through a cycle of pager “call” coverage.
