Case Manager
As a Case Manager in Northwood's Assertive Community Treatment (ACT) program, you will practice your skills in a fast-paced, non-traditional setting! This position allows you to provide creative, "old-fashioned" case management using a whatever-it-takes approach to getting things done for the people on your caseload. You will work with a supportive team and a flexible schedule. Assertive Community Treatment (ACT) is an evidence-based treatment approach that improves outcomes for people living with mental illness. Qualifications:
- Bachelor's Degree in a human service field
- At least 1 year of behavioral health or SUD experience
- Valid driver's license
- Must pass a background check
Responsibilities:
- Engaging clients in community settings
- Assisting adults with a serious mental illness to gain access to resources such as financial, housing, medical, social, vocational, educational, leisure and mental health services
- Providing services in non-traditional settings
- Participating in ACT team meetings
- Completing documentation for services provided
