Responsibilities
Job Responsibilities: This position represents a unique opportunity to provide mental health services to patients. The Licensed Professional Clinical Counselor (LPCC) works independently as well as collaboratively to manage and evaluate patient care. This position is a member of a multidisciplinary care team which collaborates with the team to meet the following patient/family/significant other needs: response to psychosocial and psychodynamic needs; coordination of community resources to ensure the transition to the least restrictive care environment; seamless continuity of care process; therapeutic intervention. The LPCC interfaces directly with patient/family/significant other and all other interdisciplinary members of the care team providing counseling and crisis intervention to patients and families identified as having complex social services needs, including but not limited to, emotional/psychiatric issues, family violence or abuse, legal issues or financial problems. The role analyzes the psychosocial and continuum of care need of patients and their progress toward desired outcome goals functioning as an educator, healthcare resource, interdisciplinary consultant and patient advocate. The LPCC provides billable services and will require proper documentation into electronic medical record system. The LPCC applies strong organizational and prioritization skills as well as highly developed problem solving and deductive reasoning skills to ensure total quality patient care. The incumbent must be dependable, self-directed, pleasant, adaptable, able to work with minimal supervisory guidance and flexible to meet patient/family and department/facility needs and be able to participate collaboratively with all team members.
Qualifications
Required:
One (1) year as a Licensed Professional Clinical Counselor
Master degree
Licensed Professional Clinical Counselor (LPCC)
Desired:
Three (3) years as a Licensed Professional Clinical Counselor
