Responsibilities
The Social Worker is a member of the multidisciplinary care team to meet the following patient/family significant other needs: response to psychosocial and psychodynamic needs; coordination of community resources to ensure the transition to the least restrictive care environment, coordination/facilitation of discharge planning, seamless continuity of care process, therapeutic intervention and financial counseling. The Social Worker interfaces directly with patient/family/significant other and all other interdisciplinary members of the care team. The Social Worker provides short-term counseling and crisis intervention to patients and families identified as having complex social services needs, including but not limited to; emotional/psychiatric issues, family violence or abuse, legal issues or financial problems.

Qualifications
Required:
- One year social work or One year social work internship
- Master Degree
- One of: CSW or LCSW or LSW
Desired:
- One year in a healthcare setting
- Master Degree
- Certified Social Worker OR Licensed Clinical Social Worker OR Licensed Social Worker
