  • Current state or compact license

Job Requirements

  • Covid vaccine required, provide vaccination card with submission.
  • Documentation of 2 MMRs at least 28 days apart and / or positive laboratory titer
  • Fit Test completed onsite annually.
  • Hepatitis B (HeQ B) vaccination series documentation or signed declination.
  • Influenza vaccine status required, declination reviewed only for religious or medical exemption.
  • 2 Negative TB skin tests required or Quantiferon Gold test.
  • Documentation of 2 doses of varicella 4 weeks apart and / or positive laboratory titer
  • Proof of Tdap vaccine required.
  • Physical / Health Statement within one year prior to assignment.
  • Norton Profile submission required.
  • Passing score of 80% or higher on Norton Specialty Exam.
  • Norton Skills Checklist to be completed prior to start.
  • Norton Drug Screen within 12 months pre-start.
  • Norton References and Employment Verification covering last year of employment.
  • Norton Background Check to be completed prior to start.
  • Norton Candidate Affirmation within 24 hours of offer acceptance.
  • Norton Employment Eligibility verification required.
  • Norton I-9 with supporting documentation completed prior to start.
  • Workplace Safety Module to be completed prior to start.
  • Norton KY TB Risk Assessment Form to be updated annually.
  • Norton Work History submission required.
  • Norton Adult Caregiver Misconduct Registry check every 3 years.
