Nursys Report preferred

COVID-19 Vaccine required

Documentation of 2 MMRs required

Fit Test required annually

Documentation of Hepatitis B vaccinations required

Influenza vaccine status required

2 Negative TB skin tests required

Documentation of 2 doses of varicella required

Proof of Tdap vaccine required

Physical / Health Statement required

Full work experience with references required

Passing score on Norton Specialty Exam required

Norton Skills Checklist required prior to start

Standard 10 panel drug screen required

Norton References and Employment Verification required

Norton Background Check required

Norton Candidate Affirmation required within 24 hours of offer acceptance

Norton Employment Eligibility required prior to start

Norton I-9 required prior to start
