Certification Details BLS ACLS Job Requirements Covid vaccine required, provide vaccination card with submission. Documentation of 2 MMRs at least 28 days apart and / or positive laboratory titer Fit Test completed onsite, completed annually. Hepatitis B vaccination documentation or signed declination for the unvaccinated. Influenza vaccine status required. Consideration for declination for religious or medical exemption only. 2 Negative TB skin tests or Quantiferon Gold within specified periods, updated annually. Documentation of 2 doses of varicella 4 weeks apart and / or positive laboratory titer Proof of Tdap vaccine received.
