What are the responsibilities and job description for the Patient Engagement Coordinator position at Norwalk Community Health Center?
Summary:
The Patient Engagement Coordinator is responsible for providing outreach services to specific populations in Norwalk Community Health Center’s (NCHC) service area. S/he helps identify and recruit patients to NCHC. The Patient Engagement Coordinator worker serves as the main contact for NCHC referral partnerships. S/he identifies community groups, churches, services groups, and other organizations and links them to NCHC services. The Patient Engagement Coordinator represents NCHC at community and local social service agency meetings and events.
Essential Duties/Responsibilities:
- Recruits individuals for program participation, workshops, community meetings, and other activities by developing and distributing outreach materials, directly contacting targeted community members, and networking with appropriate community organizations
- Conducts tour of NCHC facilities and provides information on the health center to interested parties as part of new patient orientation or to introduce individuals to NCHC
- Locates partner organizations who through collaboration with NCHC can assist our patients or potential patients
- Represents NCHC at local meetings, health fairs and other events
- Answers inquiries from community partners and helps them understand the role of NCHC in the community
- Provides participant advocacy services to assist patients in receiving needed services and to develop additional resources for services
- Attends appropriate community or networking meetings to facilitate outreach and gathering of information to support consumer access to healthcare
- Conducts small educational groups including Comenzando Bien, a five-week OB orientation· Maintains close professional relationships with local service organizations in the target area. Makes presentations regarding the services of NCHC to ensure that appropriate referrals are made to programs
- Completes participant records including eligibility documents, assessment forms, information and referral logs, volunteer and donation logs, surveys, and other records as assigned
