What are the responsibilities and job description for the Resident Services Coordinator position at Norwich Housing Authority?

Full time position. Assess the individual needs of all residents in the Authority’s State Elderly and federal complexes and work with them to establish and maintain linkages to needed supportive and social services. Maintain regular contact with residents, monitoring the delivery of services desired or required by the residents. Provide general assistance and advocacy related to services to all residents. Provide up-to-date information, clarification, and assistance regarding programs such as Medicare, Medicaid, entitlements, and formal supportive and social services. Develop and maintain linkages with community resources such as Senior Resources Agency on Aging in order to remain current regarding information and services available to address resident needs. Act as liaison with local care providers and hospitals to ensure successful discharge from care facilities and return transition of residents to their apartments. Working cooperatively with residents and their families when planning for relocation to a nursing home or other long term care facility is required. Promote a positive social climate that fosters residents’ psychosocial well-being by developing, implementing, and monitoring educational and therapeutic programs for resident participation. Provide assistance in resolution of inter-resident conflicts and resident-family difficulties. Plan employee training session to educate staff about normative aging processes, aging in place, needs of handicapped / disabled residents, service coordination and other subject areas related to main duties and responsibilities. Research and write grants and prepare monthly newsletter and written notices for residents. File accurate and timely reports to State of Connecticut and other agencies.

Must have and maintain a valid State of Connecticut driver’s license and have reliable vehicle. Prefer BS/BA degree in human or social services or related discipline or five (5) years relevant experience in a position involving direct contact with elderly handicapped and disabled individuals. Bilingual in English and Spanish preferred. Hourly pay rate of $25.00 to $26.00 for a 35-hour workweek with benefits. Must reside in the State of Connecticut and live within twenty-five (25) miles of the Authority’s main office, which is located at 10 Westwood Park, Norwich, CT. Submit cover letter and resume indicating all relevant qualifications, including three (3) work references, by April 3, 2024 close of business to: Executive Director, Norwich Housing Authority, 10 Westwood Park, Norwich, CT 06360. Candidates being considered for appointment must pass a complete post-offer medical examination, including a drug screening, prior to appointment.
