Participates in resident care conferences to assist in implementation of care plans.

- Identifies and documents changes in affect, behavior or personality. Advises the appropriate personnel.
- Assists in preparing residents and families with implementation of the discharge planning process from the facility (when possible) and
- Intervenes in all resident conflict situations as necessary.
- Assumes duties and attends meetings as directed by the Administrator.
- Assists with the management of the residents finances and with securing funding for skilled nursing care services when indicated.
- Participates in planning and providing staff educational in-services on Residents Rights, substance abuse and on the emotional/social
- Acts as a liason between facility and Norwichtown Rehab staff to foster communication and continuity of care of residents.
