Hospice Admissions Nurse.

Description

The Hospice Admission Nurse provides nursing care to hospice patients in accordance with standards of practice, state and federal regulations, and hospice protocols and procedures. The primary responsibility of this role is to facilitate the transition of patients into hospice care by conducting comprehensive initial assessments, providing detailed education and information about hospice services, and coordinating care plans. This role requires a compassionate and empathetic approach to support patients and their families emotionally during this critical time. The Hospice Admissions Nurse collaborates with a multidisciplinary team to ensure that all aspects of the patient's needs are addressed, promoting a seamless and supportive admission process into hospice care. This role requires clinical proficiency, timely and accurate documentation, and the ability to function effectively as a member of an interdisciplinary team. The Hospice Admissions Nurse may be assigned additional nursing duties such as performing hospice screens to determine patient eligibility for hospice services, hospice recertifications, coordination for GIP and / or assisting with routine hospice visits as need to support ongoing patient care and to ensure continuity of services.

Work Hours and Schedule Expectations :

• Typical workday hours are Monday through Friday from 10 : 00 AM to 6 : 00PM; this is subject to change based on patient / family needs.
• The Hospice Admission nurse is expected to participate in the emergency on-call rotation after hours, on weekends, and Holidays.

Requirements

One year acute care experience within the last two years; or

One year home health care experience within the last two years; or

One year community health or public health experience within the last two years;

One year hospice experience within the last two years.

Possesses excellent verbal and written communication skills
