Main Responsibilities
- Manage the RAI process and coordinate care plans for residents.
- Conduct thorough assessments, including physical evaluation, interview, and chart review.
- Discuss resident care needs with multidisciplinary teams.
- Analyze information from external sources and incorporate it into care planning.
- Lead PPS meetings and maintain MDS assessments per Medicare requirements.
To succeed in this role, you should have:
- Registered Nurse (RN)
- AANC certification a plus. RAC-CT
- In-depth knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
- Previous experience as an MDS Nurse
