The MDS Coordinator role provides oversight of the Resident Assessment Instrument (RAI) process and conducts assessments and care plan coordination.
This position requires strong communication and teamwork skills, as you will work closely with our experienced team to deliver exceptional care to our residents.
Responsibilities:
- Complete the Minimum Data Set (MDS), Care Area Assessments (CAA), and care plans within regulated time frames.
- Coordinate scheduling of the RAI process with the interdisciplinary team.
- Assess residents through physical assessment, interview, and chart review.
- Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review information from hospitals, consultants, and outside agencies, incorporating this information into the assessment and care planning process.
- Coordinate significant changes to MDS assessments.
- In the absence of the Care Management Coordinator, conduct PPS meetings and maintain MDS assessments per Medicare schedule, monitoring Medicare days and RUGs utilization.
- Stay up-to-date with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Qualifications:
- Registered Nurse (RN)
- AANC certification a plus (RAC-CT)
- Knowledge of the RAI process, including principles of Prospective Payment Process (PPS), strongly preferred
- Experience as an MDS Nurse
