About the Role
Ciena Healthcare seeks a detail-oriented Resident Assessment and Care Planning Specialist to oversee the RAI process and coordinate care plans for our residents.
This role requires a Registered Nurse (RN) with experience as an MDS Nurse and knowledge of the RAI process, including the principles of PPS. The ideal candidate will be able to work effectively with an interdisciplinary team and communicate resident care needs to caregivers.
Key Responsibilities:
* Conducts MDS, CAA, and care plan assessments and reviews information from hospitals, consults, and outside agencies.
* Coordinates scheduling the RAI process and assesses residents through physical assessment, interview, and chart review.
* Discusses resident care needs with caregivers and prepares for PPS meetings, maintaining MDS assessments per Medicare schedule.
* Monitors Medicare days and RUGs utilization and identifies significant change in MDS'
