About this Role
The MDS Coordinator plays a vital role in overseeing the Resident Assessment Instrument (RAI) process, conducting assessments and care plan coordination to ensure our guests receive high-quality care.
As an MDS Coordinator, you will supervise the Care Management Nurse and MDS Nurse, ensuring seamless communication and strong teamwork among our experienced staff.
Responsibilities
- Complete accurate and timely MDS assessments, CAA's, and care plans within regulatory requirements.
- Coordinate the RAI process with the interdisciplinary team, ensuring effective scheduling and communication.
- Assess residents through physical evaluations, interviews, and chart reviews, gathering essential information for care planning.
- Collaborate with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff, to discuss resident care needs and develop personalized plans.
- Review hospital information, consults, and outside agency data, integrating this knowledge into assessment and care planning processes.
- Identify and initiate significant changes to MDS assessments as needed, ensuring accurate and up-to-date information.
- Conduct PPS meetings, maintaining MDS assessments according to Medicare schedules and monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements, demonstrating expertise in RAI process and PPS principles.
Qualifications
- Registered Nurse (RN) licensure.
- AANC certification is a plus, with RAC-CT credentials strongly preferred.
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of Prospective Payment Process (PPS), is highly desirable.
- Experience as an MDS Nurse is essential for success in this role.
