Job Overview:

Nova Leap Home Health is a leading home care agency dedicated to helping older adults continue to live at home. We provide personalized home care services and solutions to fit the family.

We are seeking an outgoing, motivated and dedicated Client Care Manager (CCM) to join our team. As CCM, you will be responsible for overseeing the care and services provided to clients. This position ensures that clients receive high-quality care tailored to their needs and acts as the primary point of contact between the client, family members, and the care team. You will play a crucial role in driving business growth and ensuring the well-being of our clients. Your responsibilities include coordination of care plans, managing client relationships, and ensuring regulatory compliance, while promoting client satisfaction. You will work closely with our branch staff to develop and implement growth strategies and advance the overall quality of care delivery.

Key Responsibilities

· Client Onboarding & Assessments

o Conduct initial and ongoing assessments of clients' needs, including in-home visits.

o Develop and implement personalized care plans in collaboration with clients, families, and the care team.

o Provide guidance and support to families in understanding the services available.

· Client Relations & Support

o Serve as the primary point of contact for clients and their families, addressing concerns and resolving any issues related to care.

o Build strong relationships with clients and their families to ensure ongoing satisfaction with services.

o Act as an advocate for the client’s needs and preferences.

· Care Team Collaboration

o Collaborate with the staffing team to assign appropriate caregivers based on the client’s care plan and preferences.

o Work in conjunction with the Caregiver Manager to ensure that staff are properly implementing the care plan and providing safe and prompt care to clients.

· Quality Assurance & Compliance

o Ensure compliance with all state and federal regulations regarding home care services.

o Monitor the quality of care provided by conducting regular reviews and follow-ups with clients and caregivers.

o Document client interactions, care plans, and progress notes in accordance with agency policies.

· Service Coordination

o Coordinate services such as personal care, companionship, meal preparation, and transportation in line with the care plan.

o Adjust care plans as client needs evolve and ensure seamless communication with all stakeholders.

· Client Retention & Growth

o Monitor client satisfaction and take steps to improve services based on feedback.

o Assist in growing the client base through referrals, community outreach, and maintaining a positive reputation for the agency.

Qualifications

· Education: Bachelor’s degree in healthcare management, elder care, nursing, social work, Case management, or a related field preferred. Equivalent experience may be considered.

· Experience:

o Minimum of 3 years of experience in direct client care, case management, or similar role, preferably in home care, healthcare, or elder care.

o Experience working with seniors or chronic illnesses is a plus.

o Nursing or Social Work degree preferred, not required.

· Skills:

o Strong interpersonal and communication skills.

o Ability to handle sensitive situations with compassion and professionalism.

o Strong organizational and time management skills.

o Willingness to learn new care techniques and apply them in for the benefit of clients and families.

o Ability to work independently and as part of a team.

o Proficient in Microsoft Office and electronic health record (EHR) systems.

Work Environment & Benefits

· Work hours are Monday- Friday 8:30-4:30pm. You will spend 25% of your time in the office environment and the remaining 75% of time will be spent in the field, within the established service area meeting patients and referral sources in their home.

· On-Call Rotation will be required for this role.

Pay: $75,000.00 - $85,000.00 per year
