Job Description: As an LPN Nurse Case Manager, you will be responsible for coordinating client care, ensuring that our clients receive the highest quality of care Your role will be a full-time position for an experienced home care LPN assisting in the operational, Schedule is Monday to Friday 9am-5pm with on-call responsibilities as assigned
- Client Assessment: Conducting comprehensive assessments to determine Clients needs and developing individualized care plans.
- Care Coordination: Collaborating with healthcare providers, Clients, and their families to ensure seamless care transitions and continuity of care.
- Resource Management: Identifying and connecting clients with appropriate resources and services to support their health and well-being.
- Documentation: Maintaining accurate and up-to-date client records and documentation.
- Education: Providing education and support to caregivers, clients and their families about their care needs and assesmments
Job Type: Full-time
Pay: $85,000.00 - $90,000.00 per year
