Job Description
Title of Position: Community Liaison
Position Summary:
The Community Liaison focuses on continuity of care for patients transitioning to our
care and can make informed recommendations by maintaining knowledge of our
services and industry developments. You will promote our services by attending
conferences and seminars, cultivating relationships with referral sources, conducting
presentations, and providing clinic tours. You will explain our services to the patients,
caregivers, social workers and case managers. You will also discuss the different
levels of care that a patient may require and our role during each one of them. To
ensure success you will tactfully present the realities of processes and financial costs
when caregivers, family, and friends of patients are facing painful life events.
Duties and Responsibilities:
1. Respond to patient referrals
2. Conduct pre-screening of potential patients to determine their eligibility for
admission
3. Review medical records
4. Obtain payer information and ensure requirements are met
5. Assist with scheduling appointments or treatment
6. Coordinate patient discharges and transfers
7. Promote our services to qualified patients and their family members or caregivers
8. Implementing marketing plans to achieve sales goals.
9. Cultivating relationships with sources to generate repeat referrals.
10. Participating in the facility's marketing education, ensuring knowledge of their
resources, and maintaining knowledge of competitor providers.
11. Providing clinic tours and conducting presentations for potential referral sources
and patient families.
12. Attending seminars and conferences to promote medical groups.
13. Collaborating with facility personnel regarding referrals, evaluations, networking
and marketing activities.
14. Reviewing physicians' orders and patients' clinical needs and gathering clinical
information.
15. Communicating activities and concerns to supervisor and department
Administrator.
16. Other duties as assigned.
Qualification and Skills:
• Proven experience in healthcare Community Liaison
• Proficient in using electronic health record (EHR) systems
• Excellent organizational and leadership skills
• Ability to work well under pressure multitask and prioritize tasks effectively
• Exceptional communication and interpersonal skills
