Clinical Pharmacist (Inpatient experience in Infomatics and Data Analytics and PGY1 residency required)

Shift- 7am-4pm

Novant Health is seeking a Full-Time Clinical Pharmacist , to join our acute care pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

Essential Functions Include

• Data analytics and Informatics is required for the role
• Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
• Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems
• Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
• Makes appropriate evidence-based, patient- centered medication recommendations
• Participates in the management of medical emergencies
• Provides discharge medication review, reconciliation, and counseling as appropriate
• Maintains proficiency in and actively engages in central dispensing functions and operational programs
• Oversees pharmacy operations and technician practice/activities

Qualifications

• Education: 4 Year / Bachelors Degree, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. PhD, preferred. Doctorate of Pharmacy, preferred.
• Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
• Licensure/Certification:
• Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity., required.
• Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
• Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
