Hospice RN - On Call/Admissions
Job Summary:
- This is a combination of office and field positions, Mostly field.
- Primary functions include initial assessment of hospice patients in their place of residence (field) and providing clinical guidance to hospice patients/families (office)
- Coordinate patient care with field case managers
- Coordinate care with the interdisciplinary team, patient/family and referring agency
- Perform On Call Duties
