Essential Duties:
- Excellent communication and interpersonal skills to collaborate with healthcare providers, staff, patients, and stakeholders.
- Basic understanding of hospital operations and steps needed to create a comprehensive discharge plan.
- Coordinating and scheduling appointments for patients with multiple healthcare providers to ensure continuity of care.
- Facilitating communication and information exchange between patients, healthcare providers, and other stakeholders involved in the care process.
- Monitoring patients' progress and adherence to care plans, providing support and education to promote selfmanagement and preventive care.
- Conducting home visits or postdischarge to assess patients' living conditions, social support systems, and adherence to treatment plans.
- Advocating for patients' needs and rights within the healthcare system and community resources.
- Documenting patient interactions, care plans, and progress notes in electronic health records or care management systems.
- Participating in interdisciplinary team meetings and case conferences to ensure effective coordination and collaboration among healthcare professionals.
- Proficiency in healthcare management systems and technology, including electronic health records (EHR) and patient management software.
- Knowledge of healthcare regulations, accreditation standards, and compliance requirements.
- Commitment to patientcentered care and continuous quality improvement.
- Ability to build and maintain relationships with community organizations and stakeholders.
- Additional duties as assigned
Required education and experience:
- Minimum of 2 years of experience in a community mental health setting preferably with some experience in a hospital, crisis center, SMI clinic or behavioral health home.
- Bachelor's degree in social service field (psychology, counseling, social work, public health) or a related field (or equivalent combination of education and experience)
