NovumHealth, a multi-state Behavioral Health company is expanding its services in Salt Lake City and is in need of several professional Case Managers.
The ideal candidate with have, at a minimum, a bachelors in Social Work. Licensed Social Workers, and Licensed Therapists are strongly preferred.
Job Overview
As a Case Manager at Novum Health, you will play a vital role in providing comprehensive, patient-centered care to individuals with behavioral health and substance use needs. You will work closely with our clinical teams, providers, and patients to ensure appropriate care coordination, resource allocation, and support throughout the patient’s treatment journey. This role requires strong communication skills, a compassionate and empathetic approach to patient care, and the ability to manage multiple cases simultaneously. The Case Manager will be integral in enhancing patient outcomes and ensuring that individuals receive the necessary services and support to lead healthier lives.
Tasks / Responsibilities
- Patient Assessment & Care Planning: Conduct comprehensive assessments to understand patient needs, develop individualized care plans, and ensure appropriate services are in place to meet those needs. Regularly review and update care plans to align with patient progress.
- Care Coordination: Act as the primary point of contact for patients, their families, and external service providers to ensure that all aspects of care are seamlessly coordinated. Facilitate communication between various healthcare providers, ensuring no gaps in care.
- Resource Referral & Support: Identify and connect patients to community resources, support services, and treatment options, including behavioral health and substance use services, social support programs, and housing assistance.
- Progress Monitoring: Track patient progress, identify barriers to treatment, and modify care plans as needed. Ensure patients are meeting their care goals and receiving necessary followups.
- Patient Advocacy: Advocate on behalf of patients, ensuring they receive the best care possible and addressing any concerns they may have regarding their treatment or service delivery.
- Documentation & Reporting: Maintain detailed, accurate, and timely case notes and records in compliance with company policies, healthcare regulations, and HIPAA requirements. Provide regular reports on patient outcomes, care plan progress, and any challenges encountered.
- Collaboration & Team Engagement: Work closely with multidisciplinary teams, including clinical staff, social workers, healthcare providers, and external agencies, to ensure integrated care and treatment delivery. Participate in team meetings and case reviews.
- Crisis Management: Respond to crisis situations with appropriate interventions, ensuring patient safety and coordinating with emergency services when necessary. Provide immediate support to individuals in crisis, including referrals to urgent care services.
- Education & Training: Educate patients and their families about available resources, treatment options, and healthy coping mechanisms. Support patients in making informed decisions about their care and treatment paths.
- Compliance & Regulations: Ensure all case management activities adhere to relevant healthcare regulations, privacy standards (HIPAA), and organizational policies. Maintain a strong focus on patient confidentiality and care quality.

Education and Experience
Years of Experience
A minimum of 2-3 years in a health care setting preferably in mental health.
Education
Bachelors in Social Work or related field.
Master’s preferred in social work or counseling/therapy.
License, Certificate Or Registration
BLS (Basic Life Support)
Licensed Social Workers or Licensed therapists preferred.
