Job Description
Duties & Responsibilities:
✅ Administer medications, treatments, and therapeutic interventions as prescribed by physicians.
✅ Perform venipuncture, wound care, catheter care, and tracheostomy care as needed.
✅ Assist in patient assessments, including monitoring vitals, pain management, and chronic conditions.
✅ Provide ADL assistance, including bathing, feeding, mobility support, and repositioning.
✅ Care for medically fragile patients, including those on ventilators, G-tube feeding, or with chronic illnesses.
✅ Accurately document patient care in compliance with healthcare regulations.
✅ Collaborate with RNs, physicians, and therapists to develop patient-centered care plans.
✅ Educate patients and families on medication management, condition monitoring, and discharge planning.
✅ Maintain a safe, clean, and supportive environment to prevent infections and complications.
Required Skills & Qualifications
✅ Valid California LVN license (in good standing).
✅ Experience in CLHF, SNF, or long-term care preferred.
✅ Proficiency in G-tube care, catheterization, ventilator management, and trach care.
✅ Strong knowledge of chronic disease management, anatomy, and physiology.
✅ Ability to lift, transfer, and assist patients safely.
✅ CPR/BLS certification required.

Pay Rate: $35-$42/hour
