Job Description
- Provide primary care services to elderly patients in assisted living, memory care, and long-term care settings.
- Conduct comprehensive medical assessments, develop care plans, and manage chronic conditions.
- Collaborate with an interdisciplinary team, including nurses, caregivers, and specialists, to ensure optimal patient care.
- Utilize telemedicine and remote patient monitoring tools to provide continuity of care.
- Maintain accurate and timely documentation in the Electronic Health Record (EHR) system.
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Maine.
- Experience in geriatrics, primary care, or a related field preferred.
- Strong ability to work independently and collaboratively within a healthcare team.
- Proficiency with EHR systems and comfort using telehealth technology.
- Excellent clinical assessment and diagnostic skills.
