Position Title: Patient Care Coordinator (Pharmacy Technician)
Reports To: Pharmacist-in-Charge
Locations: NPS Pharmacy VA01 & VA02 at LGBT Life Center
NPS Pharmacy, in partnership with LGBT Life Center, is seeking a dynamic and experienced Patient Care Coordinator (PCC) to join our pharmacy team. The PCC will be instrumental in ensuring smooth pharmacy operations, maintaining regulatory compliance, and delivering outstanding patient care. This position requires strong organizational skills, a commitment to excellence, and a passion for improving patient health outcomes. Additionally, the PCC will play a crucial part in providing pharmacy services and culturally competent care to a very diverse patient population, including individuals living with HIV, at risk of contracting STD/STIs, the LGBTQ community, communities of color, and low socioeconomic status.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Assist pharmacists (e.g., PIC, Staff Pharmacist, PRN) in preparing and dispensing prescribed medications.
- Enter prescription information and/or type prescription label(s) into a pharmacy filling system (e.g., PioneerRx)
- Record prescription information meticulously into all pharmacy filling systems and databases to maintain accurate patient records.
- Ensure compliance with labeling requirements and affix appropriate labels to medication containers (e.g., vial, bottle, box, bubble pack).
- Assist in pharmacy inventory management by accurately counting, pouring, mixing, and packaging of medication, including removing expired medications.
- Answer incoming and outgoing calls to patients, case managers, providers, insurance companies (payers), nurses, doctors, healthcare providers, etc.
- Help pharmacist provide comprehensive patient care and education, ensuring patients understand their medications and treatment plans.
- Interact courteously with patients and team members to develop and maintain positive, professional relationships.
- Produce routine reports and correspondence with clarity and accuracy.
- Coordinate prescription deliveries efficiently to meet patient needs.
- Proactively identify operational challenges and provide effective solutions to improve pharmacy processes and patient care.
- Other additional related duties as assigned.
PREFERRED QUALIFICATIONS
- High school diploma or GED.
- Certified Pharmacy Technician (CPhT).
- Proficient in English with strong verbal and written communication skills.
- Strong typing and data entry skills.
- Strong Technical and MS Office skills (Word, Excel, Outlook, Zoom, etc.).
- Experience with pharmacy software (e.g., PioneerRx).
- Excellent interpersonal, customer service, managerial, and communication skills.
- Must have a positive attitude, be energetic and flexible, have excellent phone etiquette, and provide exceptional customer service.
- Ability to establish positive rapport, build relationships, and provide culturally sensitive patient care.
- Able to efficiently operate in a fast-paced environment while fostering positive customer relations.
- Ability to travel between multiple pharmacy locations in the Hampton Roads area is required.
- Must have a reliable transportation mode to and from both NPS Pharmacy locations regularly.
HIGHLY PREFERRED QUALIFICATIONS
- 2 years of full-time retail/community, clinical, specialty, or 340B pharmacy experience.
- Associate degree or higher, especially in a related healthcare field
- Extensive experience with PioneerRx
- Familiarity with Electronic Medical/Health Records (EMR/EHR)
- Experience providing direct pharmaceutical care, serving as a drug information and patient education resource, and assisting with the monitoring of patients living with the Human Immunodeficiency Virus (HIV), including:
- Experience providing comprehensive pharmaceutical support to patients living with HIV in a clinical environment and/or in partnership with a 340B covered entity.
- Has experience providing direct pharmaceutical care, serving as a drug information and patient education resource, and assisting with the monitoring of patients who identify as part of the LGBTQ community, including:
- Experience providing comprehensive pharmaceutical support to patients identifying as LGBTQ in a clinical environment and/or in partnership with a 340B covered entity.
- Currently has or is willing to acquire foundational knowledge about key concepts and terminologies relevant to the LGBTQ population, including but not limited to pronoun usage, sex, sexual orientation, gender, gender identities, and intersectionality of multiple identities.
- Knowledge and understanding of the federal 340B Drug Pricing Program.
- Bilingual in Spanish
LOCATION
This position is co-located between NPS Pharmacy VA01 (Hampton, VA) and NPS Pharmacy VA02 (Norfolk, VA), plus any additional pharmacy locations added in the region.
