Your work will directly impact patients' health outcomes by gathering crucial information about their health status, concerns, and needs.

Key Responsibilities

Conduct thorough assessment calls to gather relevant patient health information.
Identify and escalate patient needs, including prescription refill requests, appointment scheduling, and connecting patients with community resources.
Develop and implement comprehensive care plans based on patient assessments.
Perform Behavioral Health Integration (BHI) assessments to address the mental health aspects of patient care.
Provide patients with clear information about their health conditions, treatment plans, and self-management strategies.
Collaborate with patients to set and achieve realistic health goals.
Assist patients with troubleshooting medical devices and provide guidance on device setup.
Respond promptly to patient inquiries and follow-up on calls as needed.
Manage appointment rescheduling and regularly check and return voicemails.
Contribute to a collaborative and positive work environment, embodying the Nsight culture of positivity.

Qualifications

License : An active compact LPN / LVN license is required. If you are licensed in all 50 states,

Language : Bilingual Spanish required.

Experience : Passion for patient care, strong ability to connect with patients with empathy and compassion. A passion for patient education is essential.

Work Environment : Quiet, HIPAA-compliant workspace.

Skills : Moderate to advanced computer skills.
