You’ll work on the front lines with physicians and clinical teams, delivering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services.
Your Role:
As a key contributor to Nsight’s mission, you will ensure that patients have access to our innovative care solutions, helping them live healthier, longer lives.
Key Responsibilities:
- Build Relationships: Partner with clinicians and their office staff to support patient enrollments in Nsight’s RPM and CCM services.
- Drive Success: Facilitate high-value, patient-centered care by working directly in clinics to enroll patients, organize data, and enhance communication between providers and patients.
- Educate & Engage: Be the bridge between patients and clinical teams, providing education about Nsight’s care programs and driving engagement.
- Champion Change: Implement team-based, patient-centered processes that improve chronic disease management.
- Optimize Outcomes: Collaborate with care teams to ensure timely onboarding and care plan adherence.
What We’re Looking For:
- 5-10 years in customer success, account management, or client services, preferably in healthcare.
- A track record of driving patient enrollments and success in a fast-paced, client-facing role.
- Strong communication and relationship-building skills, with the ability to educate and inspire.
- Experience with HubSpot, EHR/EMR systems, and customer service tools a plus.
- Bilingual in English and Spanish is a plus!
