Job Opening: RN Case Manager – Emergency Department (ED)
Shift: Monday–Friday, 8:00 AM – 4:30 PM
Contract Type: Travel or Local Assignment
Travelers Must Reside: 50 miles from facility
Position Overview:
Join the team at the state's only Level 1 Trauma Center, as an RN Case Manager supporting our Emergency Department (ED). This is a high-impact role requiring experience in acute care management and discharge planning for complex patient populations. You’ll be part of a dynamic Care Management team committed to improving patient outcomes and ensuring safe, timely transitions of care.
Key Responsibilities:
- Manage a caseload of 15–20 patients/consults per day
- Conduct comprehensive discharge planning for ED and inpatient units
- Collaborate with the Social Work team, respiratory therapy, and pharmacy as needed
- Serve as a key liaison in the hospital’s care coordination strategy
Required Experience & Qualifications:
- Active RN License
- Minimum 2 years of acute care case management/discharge planning experience
- At least 6 months in a Level 1 or Level 2 trauma center ED
- EPIC charting experience – Required
- BLS certification
- CCM certification – Preferred, not required
- Education: BSN preferred; ADN considered
Schedule & Work Environment:
- Schedule: Monday through Friday, 8:00 AM – 4:30 PM (5x8hr shifts – non-negotiable)
- No weekend requirement
- Holidays: Rotated as needed
- Scrub/Dress Code: Any color scrubs or business professional attire
- Orientation: 2 weeks, including shadowing and general facility onboarding – you won’t be alone
Department & Team Info:
- Department: General Case Management / Discharge Planning
- Currently 15 RN Case Managers, with a goal of 21 total (18 General, 3 Complex)
- Team works closely with 15 full-time and 17 part-time Social Workers
- Reports directly to the Social Work Manager; department oversight by the Chief of Care Coordination
- No current CM Manager in place
Facility Highlights:
- UMMC holds exclusive contracts with 5 local facilities, each with 10 reserved beds to assist with complex discharges
- Special considerations apply for uninsured or underinsured patients
- Expect a diverse caseload with complex discharges and patients with no other hospital options
Job Types: Full-time, Contract
Pay: $2,546.00 per week
